* mod_auth_digest: Fix some missed "strcasecmp <--> ap_cstr_casecmp" conversions

   * mod_lua: Avoid "may be uninitialized" warning
   * ab: Fix alignment in an 'if' block.
   * mod_authz_dbd: fix select-groups example, bz 66010
