Remove most of the annoying "re ap_context_t mplement" mis-replacements
...
by what I think is their real meaning. Guys, I didn't verify all of this,
please have a close look at my "fixes". Thanks.

Changed pools to contexts. Tested with prefork and pthread mpm's. I'll
